New Clients

Thank you for your interest in The Elder Law Firm. If you have an appointment to meet with a lawyer at The Elder Law Firm, you can download and print the forms and lists for your appointment here. The forms are in the PDF format.

We ask each new client to complete a First Meeting Information Sheet and bring it to the first meeting. It provides us with basic information about you and about your case. We may also ask you to bring copies of documents and other paperwork to the meeting to help us answer your questions and handle your case efficiently. The information that we request is tied to the reason for the appointment.
